Patient Information
We need some information from you to grant a MyChart account. Once submitted, it could take up to a week for processing and verification. Once approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account. If you have any questions, please contact your clinic.
To find your MRN (medical record number), log into our old Patient Portal, and open any after-visit summary. Look for the "ID#" (note: this is your old MRN). Your NEW MRN is your old MRN with 0's (zero) added to the front of the number to make it 9 digits long (example: 001234567).

Proxy Access provides access to another person’s MyChart account. Examples of proxy access are your child’s account (under age 18), an adolescent with diminished capacity, or an adult whose care you help manage.
